By confirming your request, you consent to us disclosing your TFN to your other fund and requesting they transfer your benefit to your Nationwide Super account. You also acknowledge you are aware benefits (like insurance cover) may cease if you close your account(s) with your other super fund(s). You should ask your other super fund for information about any fees or charges that apply, or other information about the effect this transfer may have on your benefits.

• You’ve considered where your future employer contributions will be paid (if your employer contributions are currently being paid to another fund).
• You are aware that if you’ve made an after-tax contribution to your other fund, you may be eligible to claim a tax deduction. You can only claim a tax deduction before exiting the fund.
• The transfer will be requested according to the fund’s ABN or USI and the other fund may be known by a different name than shown.
